We are proud to be an approved provider of the NYS Department of Health’s Traumatic Brain Injury (TBI) Medicaid Waiver Services.
Our Service Coordinators do more than manage paperwork — they walk alongside you, your family, and your care team to make sure you have the right supports in place to live as independently and fully as possible. Whether it’s transitioning out of a nursing home, arranging respite for family caregivers, or coordinating services so you can thrive in your own home, we are here to make it happen.
How We Help
Service Coordination connects you to the resources and supports that matter most. This can include:
Securing financial benefits such as Social Security
Linking to aide agencies for daily self-care support
Finding and securing affordable housing
Coordinating medical services and providers
Connecting you with vocational or educational opportunities
Arranging transportation to medical appointments
Linking you to speech, occupational, and physical therapies
Applying for services available through the NYS Medicaid Waiver program
And the support doesn’t stop there — your Service Coordinator regularly checks in, monitors your progress, and adjusts your services as your needs change.
