At Bridges, we know that health is about more than doctor’s visits. It’s about the everyday supports that keep life steady — food on the table, a safe place to live, reliable transportation, and access to community resources. That’s why we’re proud to provide Social Care Network (FLIPA) services for Medicaid members in our region.


Who We Help

  • People with or without a disability who need extra support with daily life
  • Anyone facing challenges with food, housing, transportation, or benefits

What We Provide

With just a phone call, our team can:

  • Housing Support — Find safe, affordable housing and help with applications
  • Nutrition & Food Access — Referrals to food pantries, SNAP enrollment, and community meal programs
  • Transportation — Connections to reliable transit so you can get to work, medical care, or community activities
  • Benefit Navigation — Help with Medicaid renewals, SNAP, and other social services
  • Crisis & Ongoing Case Management — When challenges are bigger, we’ll walk alongside you with personalized support

All services are free for Medicaid members.
