That’s why we’re proud to provide Social Care Network (FLIPA) services for Medicaid members in our region.
Who We Help
- Adults and families with Medicaid coverage
- People with or without a disability who need extra support with daily life
- Anyone facing challenges with food, housing, transportation, or benefits
What We Provide
With just a phone call, our team can:
- Housing Support — Find safe, affordable housing and help with applications
- Nutrition & Food Access — Referrals to food pantries, SNAP enrollment, and community meal programs
- Transportation — Connections to reliable transit so you can get to work, medical care, or community activities
- Benefit Navigation — Help with Medicaid renewals, SNAP, and other social services
- Crisis & Ongoing Case Management — When challenges are bigger, we’ll walk alongside you with personalized support
All services are free for Medicaid members.
