Staying healthy isn’t always simple — especially if you’re living with chronic health conditions, mental health challenges, or facing barriers like housing, food, or transportation. New York State created the Health Home program to support people in exactly these situations.
The goal is simple: to help you get the care and services you need, when you need them. That means fewer emergency room visits, more support from doctors and providers, safe housing options, and reliable ways to get to your appointments.
What is a Health Home?
A Health Home isn’t a place — it’s a team. It’s a network of healthcare providers, community organizations, and Care Managers like us at Bridges, all working together to support you.
When you enroll, you’re connected with a dedicated Care Manager who will work side-by-side with you to create a personalized care plan. This plan is built around your needs and goals, helping you stay healthy and live more independently.
How We Help
Your Care Manager can:
- Connect you with healthcare providers you can trust
- Link you to mental health and substance use services
- Help you get and manage your medications
- Organize and remind you about appointments
- Assist with housing support and stability
- Guide you through social services like food, benefits, and transportation
- Connect you to community programs that bring extra support

Eligibility
In order to be eligible for Health Home services, the individual must:
- Be 21 years or older
- Enrolled in Medicaid
- Live within the following New York counties: Monroe, Ontario, Livingston, Wayne, Seneca and Yates
And have one or more of the following conditions:
- Significant mental illness or serious emotional disturbance
- HIV/AIDS
Two or more chronic health conditions (i.e. asthma, diabetes, heart disease, mental health condition, substance use disorder, etc.)
