Traumatic Brain Injury (TBI) Waiver Services

Who Qualifies: Adults with a documented traumatic brain injury who have Medicaid. We can help you apply and get connected.

Services Available:

  • Service Coordination (SC): A partner by your side to navigate systems, advocate for your needs, and make sure nothing slips through the cracks.
  • Independent Living Skills Training (ILST): Hands-on coaching in everyday skills — from cooking and budgeting to transportation — to build independence and confidence.
  • Day Program Services: A safe, supportive place to connect, learn, and grow through daily activities, rehabilitation, and meaningful peer support.

Nursing Home Transition & Diversion (NHTD) Waiver Services

Who Qualifies: Adults with disabilities who have Medicaid and are at risk of nursing home placement, or transitioning out of a nursing facility. We can help with the enrollment process.

Services Available:

  • Service Coordination (SC): Personalized planning and support to help you stay where you want to be — in your own home and community.
  • Independent Living Skills Training (ILST): One-on-one support to build everyday skills and strengthen your independence in community living.
  • Day Program Services: A welcoming program where you can socialize, learn, and enjoy purposeful activities that enhance quality of life.

Health Home Care Management

Who Qualifies: Adults 21+ with Medicaid who live in Monroe, Ontario, Livingston, Wayne, Seneca, or Yates counties and have one or more qualifying conditions (such as serious mental illness, HIV/AIDS, or two or more chronic health conditions like asthma, diabetes, or heart disease).

Services Available:

  • Care Coordination: A dedicated Care Manager to organize your doctors, medications, and appointments so you don’t have to juggle it all alone.
  • Whole-Person Support: Help connecting to housing, food, transportation, and benefits (like Medicaid renewals or SNAP) — because health is about more than just medical care.
  • Personalized Planning: A care plan built around your goals and needs, with ongoing support to help you stay well.

Social Care Network / FLIPA

Who Qualifies: Anyone with Medicaid coverage, with or without a disability, who has unmet needs such as housing, food, SNAP benefits, pantries, transportation, Medicaid recertification, and more...

Services Available:

  • Quick Phone Support: One call connects you to immediate resources like food pantries, housing assistance, or benefits.
  • Resource Navigation: Guidance to local programs and community supports that match your needs.
  • Case Management: For more complex challenges, we provide hands-on support through crisis and ongoing situations.
