Bridges For Brain Injury is excited to expand services to support veterans through a new Community Reintegration Program and is seeking a compassionate, motivated, and independent Community Reintegration & Life Skills Coach to join our team.
This position is more than a job — it is an opportunity to walk alongside someone during an important chapter of their journey, helping them strengthen independence, rebuild confidence, reconnect with their communities, and move toward meaningful goals.
Position Details:
• Initial Per Diem / Part-Time position with potential to grow into Full-Time as the program expands
• Flexible hybrid work environment
• Independent community-based position
• Travel throughout the Finger Lakes and Greater Rochester area (mileage reimbursed)
• Company phone and laptop provided
• Compensation: $22.00–$25.00 per hour, depending on experience, qualifications, and assigned responsibilities
• Initial Per Diem / Part-Time position with potential to grow into Full-Time as the program expands
• Employees moving into Full-Time status become eligible for Bridges For Brain Injury benefit offerings in accordance with organizational policies
What You'll Do:
• Provide coaching, encouragement, and support to individuals working toward personal goals and increased independence
• Help individuals strengthen routines and daily living skills
• Support community reintegration and participation
• Encourage social connection and involvement in meaningful activities
• Assist individuals in identifying and accessing community resources and opportunities
• Promote problem-solving, confidence, self-advocacy, and decision-making skills
• Maintain communication with supervisors, care managers, and team members
• Complete required progress notes and documentation
Who We're Looking For:
• High school diploma or equivalent required
• Associate’s or Bachelor’s degree in Human Services, Social Work, Psychology, Rehabilitation, Education, Counseling, or related field preferred
• Experience working with veterans, individuals with traumatic brain injury, disabilities, behavioral health, rehabilitation, mentoring, coaching, or community-based support preferred
• Strong communication and relationship-building skills
• Ability to work independently and manage schedules effectively
• Compassionate, dependable, professional, and motivated
• Valid NYS Driver's License and reliable transportation required
