What We Do

TBI/NHTD Medicaid Waiver Service Coordination

We are an approved provider of NYS Traumatic Brain Injury (TBI) and Nursing Home Transition & Diversion (NHTD) Medicaid Waiver Services. 

Service Coordination services are essential for maintaining a participant's independence as they coordinate all the community services someone could need to support them to live as independently as possible.  We have experience helping people to transition from a nursing home, adding services to provide family respite or arranging services for someone to live in their own home.  

Linkage to Services Can Range From:

  • Securing financial benefits such as Social Security

  • Linking to aide agencies for self-care assistance

  • Finding and securing a place the participant can afford to live

  • Arranging medical services

  • Connecting to vocational/educational services

  • Arranging transportation to medical appointments 

  •  Linking to speech, occupational and physical therapies 

  • Applying for services such as ours which are approved through the NYS Department of Health Medicaid Waiver program.  

The Service Coordinator then monitors the participant’s services and makes adjustments based on how well he/she is progressing
