Our Programs & Services

We offer a wide range of programs and services to meet the needs of those affected by a brain injury or physical disability. We assist them and their families by providing programs, information and resources to assist in their journey. We also foster awareness through public outreach and media.

Traumatic Brain Injury Medicaid Waiver Services

The dignity of risk and right to fail are integral parts of the Traumatic Brain Injury (TBI) Medicaid Waiver's philosophy.
The philosophy supports the participant right to choose where to live, who to live and socialize with, and what goals and activities to pursue.

ELIGIBILITY REQUIREMENTS:
1) Be recipient of Medicaid coverage that supports community based long term care services.
2) Have a diagnosis of a Traumatic Brain Injury
3) Be between the age of 18 and 64
4) Be assessed to need a nursing home level of care as a direct result of the brain injury

Nursing Home Transition & Diversion Waiver Services

The NHTD Medicaid Waiver program was developed based on the philosophy that individuals with disabilities and/or seniors have the same rights as others. This includes the right to be in control of their lives, encounter and manage risks and learn from their experiences. This is balanced with assuring the participant's health and welfare.

ELIGIBILITY REQUIREMENTS:
1) Be a recipient of Medicaid coverage that supports community based long term care services.
2) Be between age 18-64 with a physical disability or age 65 or older
If under age 65, physical disability must be documented. This could be by:
SSI or SSD Award letter
Documentation from a physician or specialist
Letter from Department of Social Services
3) Be assessed to need a nursing home level of care

TBI & NHTD Waiver Services We Offer

  • Service Coordination

    Service Coordination services are essential for maintaining a participant's independence as they coordinate all the community services someone could need to support them to live as independently as possible. This can range from securing financial benefits such as social security, finding and securing a place they can afford to live, arranging medical services, vocational/educational services transportation, therapies and of course, services such as ours which are approved through the NYS Department of Health Medicaid Waiver program. The Service Coordinator then monitors the participant’s services and makes adjustments based on how well the participant is progressing.

  • Independent Living Skills Training (ILST)

    A ILST works one on one with individuals in their home & community by developing goals they choose to become more independent in their daily activities. These goals may include: budgeting, maintaining & organizing their household, planning and coordinating medical appointments they may have, taking medications as prescribed. It may include re-learning how to plan for meals, grocery shopping and meal preparation. Many of our participants struggle with short-term memory and our staff work with them on learning strategies and techniques they can use to help compensate for their memory deficits such as implementing planning and organizational tools in their home.

  • Community Integration Counseling (CIC)

    Counseling services assist survivors & their families in overcoming difficulties & challenges they face as they adjust to the effects of their injury/disability.

  • Our Connections day program offers an opportunity to work on personal goals in a group setting. This program provides several opportunities for members to contribute to the community:

    Outside The Lines Art Program: Engages members in non-traditional artistic opportunities while connecting to the community

    Connections Café: Members run an in-house snack & beverage service

    With Out Walls (WOW) Recreation program: provides recreational opportunities every Saturday for those who want to be a part of the weekend action with their peers!

    The Wildlife Defenders: The Wildlife Defenders Program is a wildlife education outreach group run by the program members of Bridges for Brain Injury Inc. Find out more about this "one of a kind" program at our Wildlife Defenders page.

  • Postive Behavioral Intervention Services

    * This service is only available through the TBI Medicaid Waiver program at this time*

    This service assists in developing individual interventions designed to decrease the consumer’s behaviors that may interfere with his/her ability to remain integrated in the community. This service provides interventions and support to the consumer’s staff to enable them to assist the consumer in understanding and overcoming their behaviors.
