Our Services

Service Coordination

Our Service Coordinators design an individualized service plan customized to a consumer's needs.  As their advocate, the Service Coordinator can help access medical services, vocational/educational resources, housing supports, financial benefits (Medicaid/SSD) and other services. 

Independent Living Skills Training

  This service is provided to survivors in their home and community by developing goals they choose to become more independent  in their daily activities.  These goals may include:   Self-Care, Completing Tasks, Meal and Grocery Planning, Household Management, Transportation,  Money Management, Pre-Vocational Skills and much more.

Community Integration Counseling

This service assists survivors and their families in overcoming difficulties and challenges they face as they adjust to effects of their injury.

Day Program Services

Our CONNECTIONS day services offer an opportunity for consumers to work on
personal goals in a group setting. This program provides several opportunities
 opportunities for members to contribute to the community:

OUTSIDE THE LINES
Engages members in non-traditional artistic opportunities,
while connecting to the community.


CONNECTIONS CAFE 
Members run an in-house snack and beverage service.
W.O.W. ~ "With Out Walls"
This program provides recreational activities every Saturday for those who want to be a part of the weekend "action" with their peers. 

WILDLIFE DEFENDERS
A wildlife education outreach group run by the program members.

General Eligibility Requirements:


*Have a diagnosis of a brain injury
*Be eligible for Medicaid
*Be between 18 and 64 years old
*Be given a choice of living in the community or a nursing facility
