Member Driven Services
SERVICE COORDINATION: Designs an individual service plan customized to a consumer's needs. As their advocate, the Service Coordinator can help access medical services, vocational/educational resources, housing supports, financial benefits (Medicaid/SSD) and other services.
INDEPENDENT LIVING SKILLS TRAINING: Is provided to survivors in their home and community by developing goals they choose to become more indenpendent in their daily activities. These goals may include: Self-Care, Completing Tasks, Meal and Grocery Planning, Household Manangement, Transportation, Money Management, Pre-Vocational Skills and much more.
COMMUNITY INTEGRATION COUNSELING: Assists survivors and their families in overcoming difficulties and challenges they face as they adjust to effects of their injury.
DAY PROGRAM SERVICES: Our CONNECTIONS Day Services offer an opportunity for consumers to work on personal goals in a group setting. - OUTSIDE THE LINES: Engages members in non-traditional, artistic opportunities while connecting to the community.
- CONNECTIONS CAFE: Members run an in-house snack and beverage service.
- W.O.W.: Our "With Out Walls" Program provides recreational opportunities every Saturday for those who want to be a part of the weekend action with their peers!
- WILDLIFE DEFENDERS: Is a Wildlife Education Outreach Group run by the program members. To find out more about this exciting program click HERE.
